All parish ministers (volunteers, staff and clergy) are required to submit an annual background check (CORI) form with the Archdiocese of Boston. This includes liturgical ministers, faith formation, pastoral ministry, and parish leadership.

In the fall of 2022, we will be collecting CORI forms after each of the Masses on the weekend of September 10 & 11. You will be asked to complete a CORI form and present it with your photo identification. We will verify that the CORI has been completely filled out and will securely send them to the Archdiocese of Boston.

You are invited to download the current CORI form, print double-sided, and bring the completed form with you to save time. This is very helpful to us!

You may also come the Parish Office during business hours to submit your CORI forms.

Thank you for your attention to this important task.

New Archdiocesan Code of Conduct – Since February 2021

In addition to the annual CORI, the Archdiocese of Boston released a new Code of Conduct for all volunteer ministers in February 2021. You may download the new Code of Conduct and acknowledge receipt of it on this webpage. Staff members should contact Tony Carbrello for the Staff Code of Conduct.
